Here’s how to generate your LOA quickly through the Maxicare Member Gateway.


💡 What is a Maxicare LOA?

A Letter of Authorization (LOA) allows Maxicare to confirm your eligibility and coverage for a medical service.

It also authorizes Maxicare to access your medical records related to that service — helping ensure you get the right care without paying out of pocket.


✅ How to Generate Your Maxicare LOA Online

1. Visit the Maxicare Member Gateway

Go to membergateway.maxicare.com.ph.

Log in with your username and password.

  • If you’re a first-time user, click Register to create an account.


2. Go to “Health Services”

After logging in, click the Health Services tab and select the type of service you need:

  • Consult a Doctor
  • Lab/Diagnostic Test

Then choose Other Providers → Request for Letter of Authorization.


3. Fill out the request form

Complete the required details and attach necessary documents such as your doctor’s request, prescription, or referral. Make sure you indicate the Hati Health service, doctor, clinic, or hospital that is Maxicare-accredited.


4. Submit your LOA request

Click Submit. A “Thank You” page will confirm your submission.

  • If approved, your LOA (in PDF format) will be sent to your registered email within 24 hours.
  • If disapproved, Maxicare will email you with the reason for disapproval. 

You can still use Hati Health services and pay out-of-pocket albeit at affordable competitive rates. 


⚡ Pro Tips

  • Request your LOA at least two (“2”) working days before your appointment.
  • Make sure uploaded documents are clear and complete.
  • Always double-check your email inbox (and spam folder) for the LOA file.
