The Philippines has the third highest tuberculosis burden in the world,  and most Filipinos with TB don't know they have it. With close to 200,000 TB cases recorded in just the first half of 2025, and the DOH targeting 12 million Filipinos for TB screening in 2026, there has never been a more urgent time to get tested.

This guide explains the full range of TB tests available in the Philippines, with a focus on TB QuantiFERON Gold, the blood-based test that is increasingly the gold standard for detecting TB exposure, especially for BCG-vaccinated Filipinos.

What Is TB QuantiFERON Gold?

TB QuantiFERON Gold (QFT-Gold) is a blood test that detects whether your immune system has been exposed to the bacteria that causes tuberculosis. It works by measuring your immune response to TB-specific antigens, proteins found only in Mycobacterium tuberculosis, the bacteria responsible for TB.

Unlike a chest X-ray, which looks for physical changes in the lungs, or a sputum test, which looks for bacteria in phlegm, QuantiFERON Gold detects the immune memory of TB exposure, making it the most effective tool for identifying latent TB infection (LTBI), the stage where TB is present in the body but not yet active.

A single blood draw is all that's required. Results are clear: positive, negative, or indeterminate.

Why TB QuantiFERON Gold Matters More in the Philippines

Most Filipinos Have Been BCG Vaccinated, and That's a Problem for Traditional TB Tests

Nearly all Filipinos receive the BCG (Bacillus Calmette-Guérin) vaccine at birth. BCG protects infants and young children against severe forms of TB but does not prevent TB infection in adults.

The traditional TB skin test, called the Mantoux or Tuberculin Skin Test (TST), is notoriously unreliable in BCG-vaccinated populations. It frequently produces false positives, meaning people are told they may have TB when they don't, leading to unnecessary anxiety, repeat testing, and in some cases, unwarranted preventive treatment.

TB QuantiFERON Gold is not affected by BCG vaccination. Its antigens are absent from the BCG vaccine strain, so a positive result reflects genuine exposure to TB

Latent TB Is the Hidden Crisis

The DOH estimates that the actual number of TB cases in the Philippines is significantly higher than recorded figures, because many Filipinos carry latent TB with no symptoms at all. Latent TB is not contagious and causes no illness, but without treatment, it carries up to a 10% lifetime risk of progressing to active, infectious TB disease.

1. Chest X-Ray (PA View)

A chest X-ray is typically the first step in TB evaluation. It can reveal lung abnormalities, infiltrates, cavities, nodules, which are consistent with TB. It is fast, widely available, and required for most pre-employment health clearances in the Philippines.

Limitation: A chest X-ray cannot confirm TB on its own. It needs to be paired with a confirmatory test.

2. Acid Fast Bacilli (AFB) Sputum Smear

The AFB sputum smear examines a phlegm sample under a microscope to detect TB bacteria. It is one of the most widely used TB diagnostics in the Philippines and is effective when active TB is already suspected. Results are available quickly.

Limitation: AFB has lower sensitivity than molecular tests and cannot detect latent TB.

Who Should Get a TB Test?

You should consider TB testing if you:

  • Have had close or prolonged contact with a confirmed TB patient
  • Work in healthcare, BPO, hospitality, retail, or any crowded environment
  • Have had a persistent cough lasting 2 weeks or more
  • Experience unexplained weight loss, night sweats, or low-grade afternoon fever
  • Are immunocompromised — living with HIV, diabetes, or taking immunosuppressive medications
  • Are required to submit a pre-employment health clearance
  • Are a parent or caregiver wanting to protect your household
  • Have not had a TB test in the past 12 months and live or work in Metro Manila or other high-density areas

If your TB test returns a positive result, or if you have persistent respiratory symptoms, you should consult a pulmonologist. A pulmonologist is a specialist in lung and respiratory diseases who can interpret your results in clinical context, order additional diagnostics if needed, and prescribe and manage TB treatment.

Standard treatment for drug-susceptible TB is a 6-month antibiotic regimen. Drug-resistant TB now has access to a newer all-oral regimen that cuts treatment from two years to six months, with higher success rates.
