PSA – Free measures the unbound (free) fraction of prostate-specific antigen in blood. It is interpreted together with PSA – Total to refine risk assessment when total PSA is borderline—a higher free-to-total PSA ratio usually suggests benign enlargement, while a lower ratio may indicate greater concern for prostate cancer. This test supports evaluation and monitoring but is not diagnostic on its own.
